Family Medical Center Wellness Exams

At Clearstone Medical Center, we take our patient’s health seriously. This doesn’t stop after the patient leaves the office. We believe that more patient history leads to an enhance ability to better treat our patients.

Clearstone Medical Center is pleased to announce that we have partnered with Accuhealth Remote Patient Monitoring (RPM) to allow our providers access to continued health data even when the patient is away from the office.

What is Remote Patient Monitoring?

RPM is the use of digital technologies to collect and analyze physiologic patient data from one location and electronically transmit that information securely to providers in a different location.  Patient data can include vital signs, weight, blood pressure, blood sugar, and/or pacemaker information.

How Does This Help Our Patients?

This technology allows us to continually monitor (24/7/365) and respond to out of normal vital events, such as blood pressure, blood sugar, and body weight. The information is reported in our Electronic Health Records (EHR) software and flagged when out of normal. If necessary, our office will reach out to the patient to schedule an appointment to address the health concern. Continuous monitoring of vital patient information provides increased information to our providers and inherently leads to better health for our patients.

Does My Insurance Cover this Service?

Yes!  In most cases, insurance will cover most or all of this vital service.  This includes patients on Medicare!
